Tiverton Wellbeing

Our in-house Wellbeing Coaches can provide personalised one-to-one support for our patients. If you’re going through a tough time, Tiverton Wellbeing is here to listen and provide practical advice. The team works collaboratively with GPs and other healthcare professionals, community and voluntary organisations, as well as other service providers in the area.

How it works

  • You can ask for support from the service by emailing us at [email protected]. You could also be referred to us by your GP, a pharmacist or by another organisation.
  • At your appointment, we will help you explore what is important to your health and wellbeing and talk about any challenges you may be facing.
  • You will have plenty of time to talk about what matters to you. Everything we discuss will remain confidential, unless we have serious concerns about your ability to keep yourself safe, or we feel you may pose a risk to others. We would always talk to you about this first.
  • Your Wellbeing Coach may suggest things you could do to help improve your situation. They may introduce you to local activities, services and organisations who could provide additional support.
  • In your final appointment, you and the Wellbeing Coach will review your progress and achievements. The Wellbeing Coach will also help to identify ways you can help yourself in the future.

Groups that people can join immediately, are:

  • Talking Hub – a friendly, informal group to enable patients to meet new people and find out about information on local groups and activities. We often have a fun quiz, followed by activities including crafting, games, puzzles with tea and coffee on offer.
  • Goal Setting – This weekly session offers patients the space to set goals with peer support and consider solution focused ideas in relation to any problems faced.
  • Mindfulness
    • What is Mindfulness and why practice it? 
    • Mindfulness is described as maintaining moment-to-moment awareness of our thoughts, feelings, bodily sensations, and surrounding environment, through a gentle, nurturing, non-judgmental lens. 
    • Mindfulness involves acceptance, meaning that we pay attention to our thoughts and feelings without judging them—without believing, for instance, that there’s a “right” or “wrong” way to think or feel in each moment. When we practice mindfulness, we tune into what we’re sensing in the present moment rather than considering the past or imagining the future. Mindfulness can help support management of pain, stress, anxiety, low mood and living with a long-term condition. At the workshop you will begin to learn the key attitudes to bringing mindfulness practice into daily life as well as learning about a longer mindfulness course that might be of interest.

How do I access the service?

You can ask for support from us by referring yourself via a website called Joy. Click the button below to get started, then click ‘Make a referral’. Please note that you must be a registered patient at Castle Place Practice or Amicus Health (Clare House and Bampton Surgeries) to be accepted.

You could also be referred to us by your GP or by another organisation.
