Forms to be completed by a GP can either be handed into Reception or you can submit an Accurx via the I have an admin query giving as much detail as possible. The form will still need to be handed into the Practice.
Please note there is a charge for non-NHS work, e.g. holiday cancellation forms, insurance forms, fire arms forms, etc. You will be informed of the charge before the form is returned to you.
